Gravel in a garden over concrete

Hi

I'm looking to lay gravel in my garden. Its a concrete base with problems of weed. Its not 100% level and slightly cracked at few points.

I have been told to lay a membrane cover to deal with the weed problem, but do I just lay the gravel on top of the cover or am I required to prepare a layer of hardcore, sub base etc. If yes, in which order do I lay them, eg: 1: cover, 2: hardcore, 3: sand, etc etc

I also want to place stepping stones in the garden with the gravel around but would like some advice on how to do it with the cover below it etc

The fabric should be laid first, click on link below for some good stuff.

When the fabric is down you can lay the gravel directly on it. The stones should be laid on a 50mm bed of sand and cement bed using sharp sand and cement mixed at 6 to 1. This is laid directly on toip of the fabric and then the gravel can be infilled.
