Manhole cover Regulations?

I fell down a manhole a few months ago. The hole was surrounded by gravel and a cheap plastic manhole cover placed over the hole. It wasnt bedded in, cemented or anything. When i stood on the corner of the cover it moved and i fell in it (by accident). lol.

Any regulations on how they should be installed?

They need to be securely bedded in and not allow surface water penetration.
