replacing mouldy grout

Hello
I want to re-grout a couple of small areas in my bathroom which have gone mouldy over time (they are right by the shower). What's the best way to start? Can I grout over the existing, or is it best to remove old grout and put new grout in?
Have never done anything like this before so any tips would be appreciated!
