staircase wall mounted hand rail

hi, is there any rule of thumb on the height that the hand rail should be mounted at.
also should the hand rail be fixed paralell to the rake of the stair case.

handrail must be at least 0.9m above the line of the tread noses.

and yes, it should run parallel with the rake of the stairs. this isn't really a post that belongs on the plumbing thread.
