water passes into pan after flushing

I have a duel button cistern after i flush the cistern a small ammount of water drips continuously into the pan. Then after about 45 minutes you can hear the cistern being topped up for a period of about 5 seconds, this happens to both loos in the house and is intermittent, the house is 3 years old.
