Damp Appeared on Concrete Floor and Caused Wooden Floor to Rise

Hi, My wooden floor recently starting to rise in the middle, to the point where it looked like a body was underneath it. I pulled up the floor to find a damp patch in the centre of concrete, which doesnt seem to go away (photo attached). Oddly there is a void in the centre of the floor with wooden boards across it about a meter wide, but no sign of moisture in there. Not sure if thats related. Any ideas? Thanks,

you need to lay either a damp proof membrane underlay, or lay a tanking solution on the floor, I would suggest removing the whole floor and repairing it
